This plan is primarily intended to support older adults, people with disabilities, and their caregivers in disaster preparedness planning. It also includes useful information for your family, friends and other support systems. While each person's abilities and needs are unique, every individual can and should take steps to prepare for all kinds of emergencies.

Health Care Directives
An Advanced Health Care Directive enables individuals to make sure that their health care wishes are known in advance and considered if for any reason they are unable to speak for themselves.
An advanced health care directive also allows a patient to appoint a health care "agent' who will have legal authority to make health care decisions in the event that the patient is incapacitated. The agent can also be appointed this authority if the patient expressly grants it through the "Power of Attorney" or POA.
