The San Diego County Sheriff's Take Me Home Program allows you to register a loved one in a data-base for easy reference in case of wandering or any other emergency. Law Enforcement will have access to the information when they respond.

Health Care Directives
An Advanced Health Care Directive enables individuals to make sure that their health care wishes are known in advance and considered if for any reason they are unable to speak for themselves.

An advanced health care directive also allows a patient to appoint a health care "agent' who will have legal authority to make health care decisions in the event that the patient is incapacitated. The agent can also be appointed this authority if the patient expressly grants it through the "Power of Attorney" or POA.
